New Patient Information:

We look forward to meeting you on your initial visit to our offices and we want to answers as many of your questions as we can that day. In order to do so, we ask that you bring the following information listed below. If you have any questions or need any assistant obtaining medical records or exams from a previous physician or facility please contact our offices before your appointment.

Please bring the following the day of your appointment:

  • Health Insurance Card/Information
  • Insurance co-pay (cash/checks/Visa/MasterCard/Discover accepted)
  • A friend or family member to take notes/help with questions
  • A list of all medications you are taking
  • A list of your questions /concerns
  • A copy of your Imaging/Radiology exams(Please do not bring just the reports)
  • Copies of medical records relevant to your office visit
  • Completed forms
